We provide whole person care with a multi-disciplinary team who journey with the patient and their family through their final days. Supporting the individual, their family and friends in this home like setting is our priority.
Hospice Residence 24/7 care is available for individuals at end-of-life who have difficulty coping at home; who do not have caregiver support; or for those who choose not to die at home, Hospice Residence care may be an option.
Registered Nurses with specialized nursing certification CHPCN(C) and Personal Support Workers are on site 24 hours per day, seven days per week.
If an individual or their Substitute Decision Maker (SDM) expresses interest in Hospice Residence care as an option for their end-of-life care needs, a referral can be made by anyone; family member, friend or a medical expert.
The referral form must be processed and signed by a Home and Community Care Support Services Coordinator to ensure that consent for a Hospice assessment has been obtained from the individual and/or their SDM.
The Home and Community Care Support Services Coordinator assesses care needs and determines the individual/family requirements for 24/7 nursing and personal care services at the Hospice. The CCAC will then forward the referral to the Hospice.
The Hospice Residence Care Coordinator then makes a visit to the patient/family to obtain information, priority of need for admission and discusses the goals of care with all involved.
